Print a DAL Prescription
Include a completed prescription form with your packaged case.
To access a printable prescription, select the specific DAL laboratory or specialty laboratory to which you’ll be sending your case.

Package Your Case
Please be sure to securely package each model/impression to prevent damage during the shipping process. Use foam between the models/impressions in the box as well as in the bottom and the top of the box.
Enclose a completed prescription with the doctor’s name, address and phone number and the patient’s name. Close the box completely. If you choose, you may place the box in a UPS bag; however, it is not required. Attach the UPS return label to the box or bag.
If you are sending multiple cases in one box, please be sure to place each individual case in a separate plastic bag.

Overnight Shipping For Non-Local Customers
DAL provides our customers in the continental United States with guaranteed overnight pickup and delivery service directly to your office for only $8.99 per case, each way ($17.98 roundtrip). DAL receives your case before 9 a.m. the next shipping day. (Keep in mind that offices located in remote areas may require additional shipping time.)
For your convenience, we provide you with UPS pre-paid labels to send your cases to DAL. To request a supply of these labels, simply call us at 1-800-227-4142 or order shipping materials online.
